If you are sending biological samples then a biological declaration form must be completed and included with the parcel. For chemical samples a Statement for chemical and reagent compounds must be completed with the sample.
For international samples you must complete a proforma invoice for customs clearance - it is advised you included four or five copies with your samples for international shipping.
You MUST ensure that the correct hazard labels are stuck to the outside of your parcel - if you are unsure please ask Andrea Keyte who will have a stock of correct labels.
Collect paperwork from the Lab manager, arrange collection online with Fedex. You do this by goign to http://www.fedex.com/gb/ukservices/ then go to the ship tab and select 'ship online - quick and simple'. Fill out all of the details and when complete take your parcel for collection from Goods Inwards. Please ensure that the goods inwards team are aware that you have a sample for collection so that they can ensure it is collected.
